Why is a penectomy performed?

There are two main reasons for having a penectomy, which include the following: Cancer treatment and gender approval.

Surgical procedures

If penectomy is recommended for patients, this kind of surgery involves a variety of techniques and methods. This type of surgery is either total or partial.
A total penectomy is the removal of the entire penis.
In this case, surgeons make a new hole in the perineum. The perineum is the area between the scrotum and the anus.

A partial penectomy removes the end of the penis.
Both procedures may be performed under general anesthesia.
Next steps that may be needed include removal of the testicles and scrotum and lymph nodes. This surgery is usually done only for very advanced cancers.
In certain conditions, such as cancer attacking deeper tissues in the body, it may be necessary to remove some lymph nodes.

Depending on the results of evaluations and tests, if cancer cells are found, the lymph nodes are removed. If cancer cells are not seen and found, no further surgery is needed.
Lymph node testing requires an incision in the groin so that lymph nodes can be extracted for evaluation.
If the tumors are located only on the foreskin, more precise surgery, such as any of the following, may be needed :

  • Mohs surgery
  • Total surgery
  • Partial punctomy

Penile Cancer

Gender Reassignment Surgery

Gender reassignment surgery helps people regain their true identity. A penectomy may be performed as a sex confirmation surgery for a transgender woman who no longer wants to hold her penis.
The purpose of a penectomy is unique to each person. Genital surgery is only one type of treatment. Because other surgical procedures may be performed to confirm the gender of transgender people.
Some people may choose to have their penis removed only, but others may choose to have their scrotum removed.

Others may decide to have the surgery to feminize their genitals, in which case the surgeon will hold the tissue to create the vagina.
